Doula Services as a CenCal/Medi-Cal Benefit

As of January 1, 2023, the Department of Health Care Services (DHCS) has added doula services as a covered benefit for Medi-Cal members, making these essential services available for individuals during pregnancy, birth, and the postpartum period. This means that managed care plans must now include doula support for prenatal, perinatal, and postpartum care.

Doulas provide personalized emotional and physical support to individuals and families, not only through pregnancy and childbirth but also in the event of miscarriage or termination. This new benefit aims to prevent perinatal complications and improve outcomes for both birthing parents and infants. Research by Sobczak et al. (2023) found that doula care is associated with better delivery outcomes, including fewer cesarean sections, shorter labor, and reduced stress during birth. Additionally, those who receive continuous doula support report feeling more empowered, experience greater satisfaction with their birth experience, and are more successful with breastfeeding.

The Medi-Cal benefit covers:

  • One initial visit

  • Up to eight additional prenatal or postpartum visits

  • Continuous support during labor and delivery

  • Continuous support during in cases of miscarriage or termination of pregnancy

  • Two extended postpartum visits

  • Up to 9 additional visits if recommended by a physician.

Doula services can be provided virtually or in person, in a variety of settings such as homes, hospitals, or alternative birthing centers. This important step ensures that more families can access the critical care and support doulas provide during one of life’s most transformative experiences.
